RE: Announcement of Community Guide Service Providers

On May 31, 2012, SMC presented an orientation session to potential Community Guide Service Providers. After the meeting, each agency in attendance had the opportunity to complete a self assessment indicating the intent to become a Community Guide Provider agency.

On June 8, 2012, SMC reviewed each self-assessment and determined which of the 15 SMC counties that each Provider would serve. SMC is pleased to announce the following agencies have agreed to provide Community Guide services to the consumers in our 15 counties:

Agency	Counties Served
Advantage Home	Cherokee, Clay, and Graham
C and B Supports	McDowell
Destiny Management, Inc.	Alexander, Caldwell and McDowell
Easter Seals of NC	Alleghany(Children only), Jackson, Macon, Swain, Watauga and Wilkes(Children only)
Lifespan	Cherokee, Clay, Haywood, Jackson, Macon, and Swain
RHA	Alleghany, Ashe and Wilkes
Summit Support Services	Ashe
The ARC of NC	Alexander, Alleghany, Ashe, Avery, Caldwell, Cherokee, Clay, Graham, Haywood, Jackson, Macon, McDowell, Swain, Watauga and Wilkes
True Behavioral Health	Alleghany, Ashe, Avery, Watauga and Wilkes
Turning Point Services	Alexander, Avery, Graham, McDowell, Watauga and Wilkes
Universal	Caldwell, McDowell, and Wilkes

For Participants that live outside of the catchment area, Community Guide Providers will be determined on a case-by-case basis.
There will be a Community Guide Introductory Training on June 21st from 9:00am to 5:00pm – location to be announced.

SMC wants to thank all the Providers who participated and offered to provide this much needed service to our consumers.
