Your Opinions Wanted!

NAMI needs your feedback! NAMI is conducting a short, anonymous survey to help us learn more about the people we serve and how we can serve you better.

The survey consists of 50 questions covering a range of topics, mostly multiple choice, and takes only about 10 minutes to complete.

Your responses are completely anonymous (unless you choose to share your name), and will be greatly appreciated by NAMI.

Thank you for taking the time to help us learn how to serve you better.
