General Statement of the Job :
|
|
This position is responsible for carrying out critical LME functions in the local community for individuals of all ages and disabilities. This position will serve as the local LME representative. The position will work with DSS to ensure access for consumers through the Work First initiative. The position will coordinate local emergency services response during working hours. The position will meet with consumers and local stakeholders in carrying out public relations and education functions. The focus of the position is as follows: coordinate local emergency services response, complete telephonic and face-to-face assessments, triage to determine status (emergent, urgent or routine), provide emergency services as needed, determine diagnosis and appropriate level of care, begin the person centered planning process, link consumer with appropriate case manager and continue collaboration with case manager around the development and implementation of the person-centered plan, provide consumer with choice of appropriate providers.

Essential Job Functions :
|
|
This Position will function as part of SMC’s Community Based Team as a local LME presence. This role will coordinate with local providers, local stakeholders, Geriatric Team and SA Prevention Providers to identify service needs/gaps and support/develop integrate services and supports across the treatment continuum from prevention to treatment intervention accessing both formal and informal support networks. This position will also assist in providing continuity of care for high risk/high need consumers in accessing appropriate services, ensuring ongoing linkage to services and clinical assistance if local provider not available. This position may also participate in clinical assessments and consultation in the local hospital, jail or DSS. This position will support System of Care activities, disaster response and other SMC initiatives. This Position will work with community partners to provide community education regarding prevention, evidenced based practices and general education regarding mental health, substance abuse, developmental disabilities and co-occurring disorders.

Minimum Training and Qualifications :
|
|
Master’s degree from an accredited school of social work and two years of social work or counseling experience; or a master’s degree in a counseling field and two years of social work or counseling.
|
|
Physical Requirements :
|
|
The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading; visual inspection involving small defects, small parts, and/or operation of machines (including inspection); using measurement devices; and/or assembly of fabrication of parts at distances close to the eyes.Mental concentration is required in all aspects of work. This position must evaluate many different types of clients in different situations. This requires flexibility and the ability to make independent decisions.

Knowledge of Job :
|
|
Thorough knowledge of social work principles, techniques, and practices, and their application to complex casework, group work and psychosocial problems and their treatment. Considerable knowledge of family and group dynamics and a range of intervention techniques. Considerable knowledge of governmental and private organizations and resources in the community. Considerable knowledge of the laws, regulations, and policies which govern the program. General knowledge of the methods and principles of casework supervision and based on medical or psychological diagnosis. Skill in establishing rapport with a client and in client’s problems. Ability to supervise, train, or instruct lower-level social workers, students, or interns in the program. Ability to establish and maintain effective working relationships with members of case load and their families as well as civic, legal, medical, social, and religious organizations. Ability to express ideas clearly and concisely and to plan and execute work effectively.
