Title :   Community Based Clinician

General Statement of the Job : This position is responsible for carrying out critical LME functions in the local community for individuals of all ages and disabilities. This position will serve as the local LME representative with providers, stakeholder agencies and other community agencies. The position will work with DSS to ensure access for consumers through the Work First initiative. The position will coordinate local emergency services response during working hours. The position will meet with consumers and local stakeholders in carrying out public relations and education functions. The focus of the position is as follows: coordinate local emergency services response, complete telephonic and face-to-face assessments, triage to determine status (emergent, urgent or routine), provide emergency services as needed, determine diagnosis and appropriate level of care, begin the person centered planning process, link consumer with appropriate enhanced provider and continue collaboration with enhanced service provider around the development and implementation of the person-centered plan, provide consumer with choice of appropriate providers. In addition, the position will work with the QI Coordinator to foster QI within the organization and maintain the principles of continuous quality care.

Essential Job Functions : The purpose of the position is to provide support to Smoky Mountain Center (SMC) in various functions that span across all locations, disabilities, and service locations. These functions essentially include access to care, participation in person-centered planning, community consultation and acting as liaison between SMC LME and the local community and incorporate the System of Care (SOC) philosophy. This unit’s primary purpose is to work with agencies across the catchment area to assist with the epidemic of drug abuse particularly with the rising Methamphetamine use in Western North Carolina. In addition, the position will work with the QI Coordinator to foster QI within the organization and maintain the principles of continuous quality care.

Minimum Training and Qualifications : Master’s degree from an accredited school of social work and two years of social work or counseling experience; or a master’s degree in a counseling field and two years of social work or counseling.

Physical Requirements : The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading; visual inspection involving small defects, small parts, and/or operation of machines (including inspection); using measurement devices; and/or assembly of fabrication of parts at distances close to the eyes.Mental concentration is required in all aspects of work. This position must evaluate many different types of clients in different situations. This requires flexibility and the ability to make independent decisions.

Knowledge of Job : Thorough knowledge of social work principles, techniques, and practices, and their application to complex casework, group work and psychosocial problems and their treatment. Considerable knowledge of famly and group dynamics and a range of intervention techniques. Considerable knowledge of governmental and private organizations and resources in the community. Considerable knowledge of the laws and principles of casework supervision and based on medical and psychological diagnosis. Skill in establishing rapport with a client and in client's problems. Ablity to supervise, train or instruct lower-level social workers, students, or interns in the program. Ability to establish and maintain effective working relationships with members of case load and their families as well as civic, legal, medical, social and religious organizations. Ability to express ideas clearly and concisely and to plan and execute work effectively.

Salary : Grade/ Step is determined by qualification and experience of candidate. Position is full-time salaried with comprehensive benefits.
