Title :   Community Based Specialist

General Statement of the Job : The Community Based Specialist will work closely with the local Community Based Clinician to provide technical support to providers, community stakeholders and consumers around complex and high-risk cases including support for Child & Family Teams as defined by the DMH/DD/SAS. Particular effort will be made to support consumers involved with Work First and Child Protective Services, hospital consults referred for outpatient services, individuals who have had broken petitions, inpatient hospital discharges, ADATC discharges, jail discharges and TASC consumers. Partner with UM in coordination efforts and identification of consumers and providers who need higher levels of assistance. Provide support with the Geriatric & Adult Mental Health Specialty Team involved consumers with when appropriate.
Will represent the LME in the community; respond to questions, inquiries and requests for information. Work with community stakeholders regarding access and availability of services through the LME and its providers; participate in community forums and interagency collaboratives addressing MH/DD/SA matters, particularly those collaboratives which legislatively require LME participation. Provide information to community stakeholders, volunteer groups and consumers regarding MH/DD/SA issues (ex. public forums, presentations). Collaborate with community stakeholders to identify and reduce service gaps and barriers to meet consumer needs. Communicates with providers re LME specific information in partnership with QPN and QM, evidence-based practices and informal supports/resources available for consumers; identify local service gaps or redundancies; address provider questions and concerns; provide training and technical assistance to providers particularly around System of Care philosophy and Child & Family Team training. Provide state approved training to meet state provider requirements in coordination with the QPN Department and ensure adequate training opportunities for network providers in the catchment in coordination with Human Resources Department.

Essential Job Functions : Must be a good communicator and will work with sister agencies, providers, commissioner and consumers to represent the LME and help insure services are rendered as needed.

Minimum Training and Qualifications : Must have BS/BA in a human services field and two years of supervised experience working with MH/SA/DD population, prefer four years of supervised experience working with DD population. Must have annual physical, PPD and CPI training.

Physical Requirements : Normal range of motion and reasonable physical health and strength will be required. Must reach families in homes, which may not be handicapped accessible. Visual attention sufficient to read, operate a motor vehicle and to observe family interactive process is necessary. Must have driver’s license. Mental concentration is required in the planning of complex interventions, review of assessments, etc. Typewriters, computers, copiers and other office equipment will be used routinely.

Knowledge of Job : The Community Based Specialist should have considerable knowledge of social work principles, techniques and practices and their application to individual casework, group work and community problems. Knowledge of the psychosocial, socioeconomic and behavioral problems and their treatment.

Salary : Grade/Step is determined by qualification and experience of candidate. Position is full-time salaried with comprehensive benefits. Work schedule is 37.5 hours per week. This position is exempt from overtime compensation.
